360 Degree Self-Check
A 360-degree self-check turns a broad personal review into a dated worksheet. The 360 Self-Check worksheet places statements around a radial chart so you can see which parts of life deserve attention and return to the same questions later.
How the assessment works
The worksheet groups prompts into ten areas: My Body, My Spirit, My Heart, My Self-Care, My Home, My Work, My Quality of Life, My Finances, My Family and Friends, and My Mojo. Read each statement as a prompt for honest reflection, then mark the circle when it describes your current position. The chart gives you a visual record of the areas you want to revisit.
Use a repeatable review
- Set aside a quiet block and complete the statements without trying to improve the picture on paper.
- Keep the date with the completed sheet so a later review has a clear reference point.
- Choose a small number of prompts that deserve a next action, then write what you will observe before the next review.
- Repeat the exercise on the cadence that fits your planning rhythm and compare the notes, not just the shape of the chart.
The value of the self-check is the conversation it starts with yourself, a coach or a trusted colleague. Use the prompts to decide what to clarify and what to change next.
