The Leadership Excellence Program worksheet is a self-review tool. It turns broad questions about how a person works into a visible set of statements that can be revisited over time.

Use the worksheet as a four-step review
First, answer each statement honestly. Mark the circle when the statement is true and leave it open when the work still needs attention. Second, color the associated blocks in the radial chart to make the pattern visible. Third, print or save the completed page so the starting point is preserved. Fourth, repeat the review at a consistent interval and compare the observations.
Review the ten areas
The worksheet groups its statements into Personal Balance, High Productivity, Self Management, Communication, Healthy Boundaries, Quality Work, Take Initiative, Manage Up, Teamwork, and Career Path. Read each area as a prompt for a specific conversation about work habits, boundaries, communication, and development.
Turn a low score into a next action
Choose one statement that needs attention and write the smallest action that would make it observable. Decide when to revisit it and what evidence you will look for. A self-review can guide a conversation; it does not diagnose a person or guarantee an outcome.
Keep the review useful
Use the same wording when you repeat the worksheet, record the date, and note what changed in the surrounding work. Share only the parts that the person wants to discuss, especially when the statements touch personal balance or boundaries.
